The Jackie Auerbach Award Nomination Form

The Walton High School PTSA is proud to sponsor the Jackie Auerbach Award. Members of the PTSA appreciate the extraordinary contributions Walton staff members make to educate our children and enhance their overall high school experience. One of the goals of the PTSA is to ensure that these contributions are recognized. Therefore, we encourage all PTSA members to nominate a staff member deserving of this special recognition.

Criteria for the Jackie Auerbach Award:

The staff member encourages and supports extracurricular student programs that enhance and contribute to all students' positive experiences.
The staff member instills in students a desire to participate in school programs and activities.
The staff member understands the individual needs of students, encourages their talents, and fosters their self-esteem.

Nominators must be members of the Walton High School PTSA and may nominate only one staff member a year. Other PTSA members (parents, staff, students) who wish to support a nomination must also submit a nomination form, but only one nomination form is required.

NEW: Submit the nomination form only. Letters of recommendation are not required.

Please email nomination forms to waltonpresident@gmail.com. The Walton High School PTSA Executive Committee will review all nominations and award the recipient $500 and a plaque.
